  • Annual Physical

1 PER YEAR

You will be contacted via email when it is time to schedule your Annual Physical. Please ensure that you mark email communications from Beta as “Safe” to prevent them from going to your spam folder.”

  • Cardiovascular Assessment

1 PER YEAR

Your Cardiovascular Assessment will be provided at the onset of your annual physical appointment. Please book your annual physical to complete this assessment. We ask that you arrive 20 minutes early to your appointment to allow enough time for this assessment.

  • Mental Health Assessment

1 PER YEAR

You will receive your comprehensive Mental Health Assessment via email. This assessment may take anywhere from 15-60 minutes to complete. Please ensure you set some time aside and find a quiet place to answer all the questions. Note that if you exit partway through, your answers will not be saved and you will need to re-start the assessment.
